Heyl Evaluator Process and Guidelines

About the Process

Thank you for your interest in submitting an evaluation for a Heyl Scholarship applicant. This year a link to the evaluation form will be emailed to you once the applicant submits their application. To keep all submissions accounted for, the foundation asks that you follow the link in your personalized email to complete the form.

Are you trying to prepare your evaluation but haven’t received your personalized email yet? Here is a preview of the questions that will be asked when you receive your link to the evaluation form. Please note that comments should focus on the student’s academics and academic habits. Evaluators are encouraged to include examples that demonstrate the student’s character as well as resilience/fortitude.

Applicant Information

  • Applicant name (auto filled)

  • Specification of where the applicant wants to attend for a Heyl Scholarship (auto filled)
  • How long you have known the applicant
  • Description of the extent and quality of your relationship to the applicant.

Academics

  • Academic Readiness – Heyl Scholars must maintain a 3.0 college GPA. In terms of maturity, academic preparation or character, do you believe this applicant will have any difficulty in studying science, math or nursing at the undergraduate level? If yes, please explain.
  • Reflection of Grades – Do you believe that this applicant’s grades accurately reflect their abilities? (Please explain, if appropriate.)

Special Ability and Interest

  • In what ways has the applicant demonstrated special ability and interest in science and/or math (including health science for nursing applicants)? Please be as specific as possible.
    • Describe within a school setting and/or outside of school
  • Potential for Contribution – What factors lead you to believe it is possible that this applicant will make a worthwhile contribution to their intended field of science, math, or the profession of nursing?
  • Additional comments (optional, but encouraged)

Letter of Recommendation

  • Post-secondary college instructors and employers please upload a letter that speaks to the applicant’s performance (academically and/or as an employee) and potential for future contributions to the chosen field. Examples that demonstrate the applicant’s character as well as resilience/fortitude are highly regarded. The uploaded letter should include the date and the evaluators signature.
    • All letters of recommendation should be submitted as a PDF.
